Position Summary
The Patient Care Advocate serves as a central liaison in the patient care journey, supporting high-touch services and managing referral pipelines and insurance navigation. This full-time, partially patient-facing role is based at the Central Business Office and plays a vital part in coordinating communication between patients, providers, and external partners. The position ensures a seamless experience from initial contact through treatment initiation.
Key Responsibilities
• Serve as the primary point of contact for patients engaging with ancillary services.
• Educate patients on available services and guide them through consultation scheduling and onboarding.
•Coordinate insurance workflows, including HMO referrals and authorizations, by liaising with PCPs and external clinics.
• Manage the lead lifecycle from referral to treatment initiation in collaboration with NP Leads, Schedulers, and the Referral/VOB team.
• Maintain accurate and timely documentation across Salesforce, Athena, and other internal platforms.
• Achieve daily call volume targets (60–80 calls) while maintaining high engagement quality and lead qualification standards.
• Support marketing and outreach initiatives, including provider relationship development and lunch-and-learn events.
• Partner with Care Navigator and Patient Advocate teams to ensure smooth execution of orders and continuity of care.
•Oversee referral management from intake to provider scheduling, ensuring timely coordination and documentation.
•Communicate with referring providers and internal teams to gather necessary information and facilitate care transitions.
• Conduct outbound calls to confirm referral details, provide updates, and guide next steps.
• Monitor referral status in Salesforce and follow up proactively to prevent delays in care.
Required Skills & Attributes
• Excellent verbal and written communication skills, with strong listening and inquiry abilities.
• High level of empathy and professionalism, especially in sensitive or complex situations.
• Detail-oriented, organized, and self-driven with a proactive approach.
• Customer-focused with a results-oriented mindset.
• Comfortable working in high-volume environments and using CRM systems (Salesforce preferred).
• Familiarity with VOB logic and insurance processes is a plus.
• Team-oriented with a coachable attitude and a commitment to continuous learning.
Education & Experience
• Bachelor’s degree or equivalent experience required.
• 0–2 years of experience in sales, customer service, lead generation, or healthcare navigation.
Work Environment & Physical Demands
• Office-based role with extensive phone and computer use (approximately 70–75% of the workday).
• May involve occasional light lifting (up to 25 lbs) and travel within the DFW area for provider meetings.
• Requires close visual acuity for screen-based tasks and documentation.
